Part 1
The episode opens with a dire situation as Lizzy goes into labor, prompting Tanya to make the risky decision to head to Cain hospital, despite the known dangers. Meanwhile, in a separate location, Michael, Datu, and Victor are deep within a tunnel, desperately searching for a way out. As they venture further, they discover strange handprints covering the tunnel walls, a disturbing sign of what has come before. The tunnel unexpectedly splits, and Datu uses his unique ability to read markings to choose a direction. They soon encounter the undead within the tunnels, forcing them to be cautious and strategic. Victor breaks a bottle of cologne as a means to mask their scent from the infected. The group continues through the tunnels, noticing that they are now inclining towards the surface. They stumble upon a cracked roof that provides a glimpse of the sky, but ultimately decide the opening is too small to exit through. They then reach what appears to be a man-made structure, complete with conduits and power boxes, leaving them questioning where they are. In the episode's closing moments, they find a stairwell and start heading upwards, encountering more and more debris as they do, and Datu finds a "Go Eels" foam finger, leading the group to realize that they are right back where they began, inside the arena.
Part 2
Episode two begins with Datu, Victor, and Michael in the basement of the arena, a location which is in terrible shape. As the group attempts to find a path to escape they are stymied by blocked and destroyed sections. The group takes Datu's suggestion to head up a floor in hopes that they may find a way to orient themselves. Eventually, the group finds a gap in a doorway and they squeeze through one at a time, heading for stairs that may lead them out. Finding themselves in a hallway, they pass multiple sealed locker rooms that, for the time being, remain unopened. Soon, the group happens upon a mass of bodies, in the pile they recognize Angel and Riley, two survivors they had thought were long gone. Even more concerning, Datu points out a trail of blood leading into a nearby dark corridor, while the trail may signify an exit, the darkness and implications of what led to it force the group to move on. Eventually the group finds the entrance to what seems to be a "Private club room" and enter with trepidation. Upon entering, they discover a horrifying scene: blood trails leading to various beds and recliners, with the scattered remains of bodies, the bulk of the remains are piled in a corner. Datu finds numbers scrawled onto the floor and as they count them they land on thirteen in total. Finding their situation even more dire, they move closer to a recliner where they find a mummified corpse of a woman, and in black ink across her forehead they see the number thirteen. Finding their situation even more dire, Michael realizes what this implies, they are in "the incubation facility" where the "little ones" were created, he then also understands the meaning behind the big ones and all the information that Tanya had gathered previously. Having discovered the secrets of the facility, Michael starts taking pictures, but all the others decide that it is time to leave this horrific place.
Part 3
Michael, Victor, and Datu, attempt to leave the facility and find a new way out of the arena. On their way out of the incubator room Datu suddenly starts feeling the affects of whatever experiments and horrific acts occurred there, a reminder of the dangers that they face, and he collapses. Knowing that he has been psychically attacked and that he is doomed and that they must not allow him to turn, Datu pleads with Michael to end his life quickly. With great reluctance and sorrow, Michael honors his friend's request and injects him with the syringe they had acquired. Datu, smiling, states that he sees Samantha before passing away.
Meanwhile, back at the hospital, Saul and Kelly continue to run through the parking structure, trying to evade the behemoth and circle back to Lizzy and Tanya. Suddenly Tanya, through the radio, is able to get off a piece of good news, she tells them Lizzy's water has broken and that a c-section will no longer be needed. Outside in the parking structure, the Behemoth is smashing cars in its relentless pursuit. Suddenly, and unexpectedly, Kelly reveals to Saul that she has a "desk job", Saul, knowing her past, questions if this is really true, leaving the audience wondering about Kelly. As they find the stairwell CJ contacts them through the radio and tells them that the helicopter is ready. Back at the hospital, with all the others in tow, they arrive on the floor where Lizzy and Tanya are, but the horde breaks into the room before they can escape, forcing them to once again move. With the hallway and stairs in sight, they know the roof and rescue are close at hand. On the helicopter, Tanya is struggling to keep Lizzy's hemorrhaging under control, but she is rapidly loosing too much blood. As the group reaches the rooftop and enters the helicopter, the behemoth, still pursuing them, digs it's fingers into the back of the cabin. Knowing they need to act quickly, Saul uses a massive muscle relaxant to subdue the Behemoth, causing it to fall from the helicopter. But, even as the Behemoth falls from the helicopter, tragedy strikes as Lizzy's condition worsens rapidly, and despite Tanya's best efforts to save her, Lizzy passes away as she holds her newborn son, Nicholas, in her arms. Saul, overcome with grief, refuses to accept her death and continues CPR, even after Tanya confirms that Lizzy is gone. As the helicopter flies away, carrying the remaining survivors, Saul's cries echo through the night, leaving all in despair.
